Between 30 and 79
Biological sex at birth
mg/dL — find this on your most recent lipid panel in your patient portal or lab printout. It is usually labeled Total Cholesterol.
mg/dL — on the same lipid panel, labeled HDL or HDL-C. This is the "good" cholesterol — higher is better.

Don't have your cholesterol numbers? This is a good reason to request a lipid panel at your next visit. Ask your provider for your total cholesterol and HDL — then come back and calculate your risk. Most patients can access recent lab results through their patient portal.

The top number of your blood pressure reading — e.g. if your BP is 128/82, enter 128. Use your most recent reading from a provider visit, home monitor, or pharmacy cuff.
Any medication prescribed to lower your blood pressure — including lisinopril, amlodipine, metoprolol, hydrochlorothiazide, and others.
Type 1 or Type 2 diabetes — including if you are on diabetes medication or were told your blood sugar is diabetic range.
Current cigarette smoker — not former smoker. If you quit, select No.
Body Mass Index. Don't know yours? Search BMI calculator online — you'll need your height and weight. Or ask your provider — it is calculated automatically at every visit.
A kidney function number from your metabolic panel — labeled eGFR or GFR. Normal is 60 or above. Leave blank if unknown — the calculator will assume normal kidney function.

Don't have your BMI or eGFR? Your BMI is calculated at every provider visit and is in your chart. Your eGFR is on your most recent basic or comprehensive metabolic panel. Both are available in your patient portal. The calculator will run without eGFR — but including it improves accuracy.
